Job Description:

This role offers an exciting opportunity to join our team at Northern Lincolnshire and Goole NHS Foundation Trust as a Community Care Coordination Specialist. The successful candidate will be responsible for ensuring that diagnostic investigations are requested and available before clinic day, and for accurately recording clinic outcomes.

Key Responsibilities:
  • Request and track diagnostic investigations
  • Record accurate clinic outcomes
  • Liaise with patient records staff, ward clerks, and other organisations to ensure availability of medical notes and referral letters
Requirements:
  • SRA III or equivalent administration experience
  • GCSE or equivalent in English and Maths grades A-C or equivalent
  • NVQ level II in business administration/customer care or acquired equivalent experience
