Community Care Specialist

We are looking for an experienced Care Coordinator to join our intake team, responsible for managing intake processes and developing relationships with external stakeholders. This is a fantastic opportunity to develop your skills and expertise in care coordination and contribute to the growth and success of our business.
About the RoleThe Intake Coordinator will be responsible for understanding the capacity of our branches' carer capacity to safely assess and accept new client referrals for our Local Authority Contracts. You will also be responsible for building and maintaining relationships with external stakeholders to support business growth.
Key Responsibilities- Manage intake processes to ensure efficient assessment and acceptance of new client referrals
- Develop and maintain relationships with external stakeholders
- Contribute to the growth and success of our business
To be successful in this role, you will need to be an efficient, organised, and experienced Care Coordinator who is passionate about providing the best in community care. You should have excellent communication and interpersonal skills, with the ability to build strong relationships with care workers and external stakeholders.
What We Offer- A competitive salary of £30,000 - £35,000 per annum (dependent on location)
