Job Overview

Stockport NHS Foundation Trust is seeking a skilled Community Health Practitioner to join our team. The successful candidate will work with Community Nurses and Adults Social Care staff to provide high-quality care to residents of Stockport.

Main Responsibilities
  • Provide care in patients' homes and residential homes for patients with complex needs.
  • Work collaboratively with the Community Nursing Service to deliver a first-class service.
  • Assess and develop care plans to meet the needs of patients.
Requirements
  • Good organisational and assessment skills.
  • Flexibility to cover the service between 08:00 and 16:00.
  • Ability to work independently and as part of a team.
