About the Role

We are seeking an experienced and skilled Community Health Specialist to join our team at Wiltshire Health and Care LLP. As a Specialist Community Nurse, you will play a key role in delivering high-quality care to patients in their homes and community settings.

Key Responsibilities
  • Lead a team of healthcare professionals in delivering community-based care services
  • Develop and implement care plans that meet the needs of patients with complex health conditions
  • Work closely with GPs, other healthcare professionals, and community services to ensure seamless care delivery
  • Conduct assessments and provide care to patients in their homes and community settings
  • Collaborate with other teams to identify and address health needs of the local population
  • Develop and maintain relationships with patients, families, and carers to ensure person-centered care
  • Participate in quality improvement initiatives to enhance care delivery and patient outcomes
Requirements
  • Registered nurse with postgraduate community qualification
  • Experience in community nursing, case management, and leadership
  • Excellent clinical skills, communication skills, and ability to work under pressure
  • Ability to work flexibly, including weekends and evenings
  • Access to a vehicle for business purposes and a full and valid driving license
