About the Role

We are seeking a highly skilled and motivated Community Health Specialist to join our team at Wiltshire Health and Care LLP. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to patients in their homes and community settings.

Key Responsibilities
  • Lead a team of healthcare professionals in delivering community-based care services
  • Develop and implement care plans that meet the needs of patients and their families
  • Work closely with GPs, other healthcare professionals, and community services to ensure seamless care delivery
  • Conduct assessments and provide care to patients in their homes and community settings
  • Collaborate with other teams to identify and address health needs of the population
  • Develop and maintain relationships with patients, families, and carers to ensure person-centered care
Requirements
  • Registered nurse with a postgraduate community qualification
  • Experience in community nursing and leadership roles
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of community health needs and population health management
