About the Role

We are seeking an experienced and skilled Community Health Specialist to join our team at Wiltshire Health and Care LLP. As a Specialist Community Nurse, you will play a vital role in delivering high-quality care to patients in their homes and community settings.

Key Responsibilities
  • Lead a team of healthcare professionals in delivering community-based care services
  • Develop and implement care plans that meet the unique needs of patients
  • Work closely with GPs, other healthcare professionals, and community services to ensure seamless care delivery
  • Conduct assessments, provide education, and support patients and their families
  • Collaborate with the multidisciplinary team to identify and address health needs of the population
  • Develop and maintain relationships with community partners to enhance care delivery
  • Participate in quality improvement initiatives to ensure high standards of care
Requirements
  • Registered Nurse with a postgraduate community qualification
  • Experience in community nursing, case management, and leadership
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of community health needs and population health management
  • Proficiency in using electronic health records and other healthcare software
