Job Description

Empower Family Group is seeking a highly motivated and enthusiastic Supported Living Support Worker to join our team in Somerset. As a Supported Living Support Worker, you will play a vital role in supporting young people aged 16-18 who are leaving care and transitioning towards independent living.

Main Responsibilities:

  • Provide practical, personal, social, and financial support to young people to help them develop their independence and confidence.
  • Complete weekly and monthly reports, support plans, and risk assessments to ensure accurate recording and planning.
  • Identify risks and concerns, and work with the team to develop plans to ensure young people's safety.
  • Support young people to re-engage with education or learning, or take the first steps towards employment.
  • Ensure the placement appearance is maintained to a high standard, including daily cleaning.
  • Help young people enjoy affordable and sustainable activities in their local community.
  • Use creative and inclusive approaches to support young people's development and improve their aspirations through 1-1 key work sessions.
  • Liaise with external agencies and professionals.
  • Adhere to policies and procedures to safeguard young people from harm and abuse.
  • Manage challenging behaviour and difficult situations.

Requirements:

  • Full UK driving licence.
  • Right to work in the UK.
  • GCSE or equivalent.
  • NVQ Level 3 qualification in Health & Social Care (preferred).
