About the Role

We are seeking a highly skilled and motivated mental health professional to join our Primary Care Mental Health Team within the Leeds Mental Wellbeing Service.

The successful candidate will be responsible for providing access to triage, specialist mental health assessment, risk assessment, and brief intervention to prevent deterioration in mental health requiring a referral to secondary mental health services.

They will also provide access to triage, assessment, and decision support for primary care colleagues and community mental health teams to ensure that service users with complex mental health needs access the correct pathway and intervention to meet their needs.

Key Responsibilities
  • Provide specialist mental health assessment and risk assessment to service users with complex mental health needs.
  • Develop and implement effective care plans to prevent deterioration in mental health and promote recovery.
  • Work closely with primary care colleagues and community mental health teams to ensure seamless service delivery and effective communication.
  • Provide supervision, support, and training to colleagues to ensure high-quality service delivery.
  • Contribute to the development of networks with staff working in other agencies that provide services to service users with complex mental health needs.
Person Specification

We are looking for a highly skilled and motivated mental health professional with:

  • Relevant core professional qualification and registration (e.g., RMN, RN (Mental Health), OT, Social Worker).
  • Extensive experience of working in partnership with service users, carers, and other agencies.
  • Ability to undertake holistic mental health and risk assessment with service users with mental health needs, formulate effective care plans, relapse prevention interventions, and safety plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with colleagues and service users.
  • Ability to adapt to barriers in communication that service users may present with emotional, psychological, and physical needs.
