Mental Health Case Manager

We are seeking a highly skilled Registered Mental Health Nurse to join our team at Service Care Solutions Ltd. As a key member of our team, you will be responsible for managing caseloads, conducting assessments, and developing care plans for Older Adults with mental health issues.
- Must hold a PIN number as a registered mental health nurse
- Fully enhanced valid DBS
This role involves:
- Managing caseloads and conducting assessments, reviews, interventions, and discharges with relevant documentations
- Conducting multi-disciplinary reviews in the community and for inpatients on Mental Health Act sections
- Liaising with the multi-disciplinary team and external agencies to support patient care
Estimated Salary: £28-£32 per hour (dependent on location)
