About the Role

Responsibilities
  • Completing Care Act assessments and mental capacity assessments in line with legislation.
  • Developing comprehensive discharge plans to meet individual patient needs.

Requirements
  • Post-qualifying experience of working within a social care setting in the UK.
  • HCPC registration and eligibility to work in the UK.
  • Enhanced DBS/CRB certification.
