About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Older Adults Community Mental Health Team (CMHT) based in the North and West of Oxfordshire.

Key Responsibilities
  • Offer assessments and interventions to patients with mental health needs, managing a caseload of patients with various mental health conditions.
  • Work collaboratively with a multi-disciplinary team, including Medical, Nursing, Occupational Therapy, and Psychology colleagues, to deliver high-quality patient care.
  • Participate in supervision and contribute to the well-being of the team.
  • Develop and maintain strong relationships with internal and external partners, including GPs, district nursing, therapy services, and social care providers.
Requirements
  • Registered Professional Qualification (Mental Health Nurse or Occupational Therapist).
  • Evidence of Continuing Professional Development (CPD) portfolio.
  • Experience working within a multidisciplinary team, including formal and informal teaching, supervising student placements, and relevant experience in community care.
  • Excellent English verbal and written communication skills, as well as IT literacy.
