Community Mental Health Practitioner

We are seeking a highly skilled and dedicated Community Mental Health Practitioner to join our Older Adult Team in North & West Oxfordshire. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs and/or dementia.
Key Responsibilities- Assess and offer interventions to individuals over the age of 65 with complex mental health needs and/or dementia.
- Work closely with the wider multidisciplinary team to deliver well-coordinated patient care.
- Develop and maintain strong relationships with internal and external partners, including GPs, district nursing, therapy services, social care, and physical and mental health inpatient providers.
- Participate in regular team meetings and contribute to the development of care plans and strategies.
- Registered Professional Qualification (RMN or OT) and evidence of Continuing Professional Development (CPD) portfolio.
- Experience of working within a multidisciplinary team and relevant experience in community care.
- Specialist knowledge and clinical expertise in community mental health in line with professional requirements.
- Excellent written and verbal communication skills, as well as good IT skills.
- Ability to work flexibly, including occasional weekend and evening shifts.
