About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner to join our Older Adults Community Mental Health Team at Oxford Health NHS Foundation Trust.

Key Responsibilities
  • Work as part of a multi-disciplinary team to provide assessments and interventions to patients with mental health needs.
  • Manage a caseload of patients with a variety of mental health conditions, including functional and organic conditions.
  • Support the duty and triage function, working closely with colleagues to ensure seamless care delivery.
Requirements
  • Registered Professional Qualification (Mental Health Nurse or Occupational Therapist).
  • Evidence of Continuing Professional Development (CPD) portfolio.
  • Experience of working within a multidisciplinary team, including formal and informal teaching and supervising student placements.
  • Relevant experience in community care and specific clinical expertise in mental health.
