Job Summary

Eastbourne East PCN is seeking a skilled Family Health Practitioner to join our team. As a key member of our primary care team, you will be responsible for providing high-quality patient care and contributing to the overall success of our organization.

Key Responsibilities

  • Conduct thorough consultations with patients to assess their health needs and develop personalized care plans.
  • Manage a range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guidelines and best practices.
  • Screen patients for disease risk factors and early signs of illness, and participate in the management of chronic diseases such as diabetes, hypertension, asthma, and COPD.
  • Develop care plans for patients in consultation with them and in line with current practice disease management protocols.
  • Provide counselling and health education to patients, and record clear and contemporaneous consultation notes to agreed standards.
  • Collect data for audit purposes and compile and issue computer-generated acute and repeat prescriptions.
  • Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
