Community Respiratory Physiotherapist

We are seeking a highly motivated and experienced Respiratory Physiotherapist or Nurse Specialist to join our Community Respiratory Service in Hounslow. As a key member of our team, you will be responsible for providing high-quality care to patients with chronic respiratory conditions, promoting self-management and preventing unplanned hospital admissions.
Main Duties- Contribute to the care provision and review of respiratory patients, supporting a multi-disciplinary approach to delivering individualized care and promoting recovery.
- Provide direct care for patients with respiratory conditions in the community-based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation and early hospital supported discharge.
- Support patients to manage their breathlessness through exercises and improve their functional abilities.
- Lead airway clearance and breathlessness management clinics.
- Provide primary care education and support.
- Develop and implement individualized care plans to meet the needs of patients with chronic respiratory conditions.

Person SpecificationWe are looking for a candidate with:
- RGN or Physio Qualifications
- Completion of the Assessors training
- Recent experience within respiratory care either in community or hospital
- Motivated to further personal and professional development
