About the Role

We are seeking a highly motivated and experienced Respiratory Physiotherapist to join our Community Respiratory Service in Hounslow. As a key member of our team, you will play a vital role in providing high-quality care to patients with respiratory conditions, promoting their independence and improving their quality of life.

Key Responsibilities
  1. Contribute to the care provision and review of respiratory patients, minimizing risks and promoting recovery through a multi-disciplinary approach.
  2. Provide direct care for patients with respiratory conditions in the community-based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation and early hospital supported discharge.
  3. Support patients to manage their breathlessness through exercises and improve their functional abilities.
  4. Lead airway clearance and breathlessness management clinics, providing education and support to patients and their families.
  5. Develop and implement primary care education and support programs to promote respiratory health and prevent hospital admissions.
  6. Collaborate with the multi-disciplinary team to develop and implement individualized care plans, promoting patient-centered care and improving health outcomes.

Person Specification

We are looking for a candidate with the following qualifications and experience:

  • RGN or Physio Qualifications
  • Completion of the Assessors training
  • Post registration nursing course in respiratory care (desirable)
  • Knowledge and understanding of health care needs in a multicultural society (desirable)
  • Knowledge of health promotion activities and strategies (desirable)
  • Recent experience within respiratory care either in community or hospital
  • Experience of working with / in primary care (desirable)
