">We are a locally-based charity dedicated to providing compassionate care for individuals living with life-limiting conditions in South West Essex.
As part of our Hospice Community Services team, we are seeking a highly motivated and experienced Registered Nurse to lead a team of Community Nurses in delivering palliative and end-of-life care in the community.
About the Role
The successful candidate will be responsible for managing a team of Community Nurses to ensure high standards of care are provided to patients and their families/carers. This includes leading by example, supporting the team, and promoting collaborative working with other healthcare professionals and agencies.
Key Responsibilities
- Leadership and Guidance: Provide leadership and guidance to the team in Hospice Community Services.
- Care Delivery: Support the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community.
- Collaborative Working: Commit to collaborative working, actively supporting and liaising with other healthcare professionals and agencies.
- Quality Assurance: Ensure high-quality care is delivered across Hospice Community Services, supporting the patient journey to ensure appropriate and seamless care.
- Audit and Research: Assist in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate.
- Team Performance: Monitor and evaluate team performance, providing guidance and support where necessary.
- Continuous Improvement: Foster a culture of continuous improvement and professional development within the team.
Requirements
- Qualifications: You will be a Registered Nurse with demonstrable post-qualification experience, including palliative care or related specialties.
- Skills and Experience: Excellent communication and interpersonal abilities, as well as a sound knowledge base linked with holistic assessment and advance care planning. You will have a passion for making a difference in the community.
- Additional Requirements: Have experience in assessment skills and non-medical prescribing or have willingness to undertake these. Experience in supervising junior staff and pre-registration students.
Salary and Benefits
The estimated salary range for this role is £23,572 - £29,925 per annum (£39,287 - £49,875 full-time equivalent). We offer a competitive benefits package, including opportunities for professional development and a supportive work environment.
