As a Community Nurse Team Leader at St Luke's Hospice, you will have the opportunity to make a real difference in the lives of patients and their families. Our team provides compassionate care to individuals living with life-limiting conditions in South West Essex, and we are seeking a highly motivated and experienced individual to lead our Community Nurses.

About the Opportunity

In this role, you will be responsible for leading a team of Community Nurses, ensuring the provision of high-quality palliative and end-of-life care in the community. You will work closely with other healthcare professionals and agencies to provide seamless care, ensuring that patients receive the right care at the right time.

Responsibilities:

  • Leading a team of Community Nurses in Hospice Community Services.
  • Supporting teams to deliver high-quality care to individuals with palliative care needs.
  • Collaborating with other healthcare professionals and agencies.
  • Ensuring high-quality care is delivered across Hospice Community Services.
  • Contributing to audit and research projects.
  • Monitoring and evaluating team performance.
  • Fostering a culture of continuous improvement and professional development.

Requirements:

  • Registered Nurse with post-qualification experience in palliative care or related specialties.
  • Excellent communication and interpersonal abilities.
  • Sound knowledge base linked with holistic assessment and advance care planning.

Salary: £23,572 - £29,925 (£39,287 - £49,875 per year full-time equivalent)

Contract: Permanent
Hours: Part-time, 22.5 hours per week, Monday to Friday
