Job Title: Care Coordinator for Vulnerable Families

The salary for this role is up to £43,839 per annum. As a Care Coordinator, you will work closely with families who require support due to complex social care needs.

Main Responsibilities:
  • Assessment and Planning: Conduct thorough assessments of family functioning, providing targeted support to prevent relationship breakdowns that may threaten the health or wellbeing of children at risk of abuse or neglect.
  • Collaboration and Partnership Working: Develop effective relationships with professionals, both internally and externally, to improve the lives of children, young people, and families in need.
  • Child Protection and Safeguarding: Lead assessments and plan responses to safeguard children from abuse, neglect, or exploitation, ensuring the well-being of all parties involved.
  • Alternative Care Arrangements: Collaborate with families and relevant authorities to arrange alternative care arrangements when necessary, prioritizing the best interests of the child.
  • Supporting Vulnerable Individuals: Promote autonomy and development among individuals with complex social needs, such as those with learning disabilities, physical disabilities, or diminished capacity.
  • Record Keeping and Participation: Maintain accurate and high-quality records of activity on cases and participate in planning and decision-making forums, including case discussions, Strategy Meetings, and Child Protection Conferences.
