Palliative Care Coordinator

St. Luke's Hospice, a charity rooted in the local community, is seeking an experienced and devoted registered nurse to join its Innovative Hospice Rapid Access Service. This role offers a rewarding opportunity for someone passionate about delivering high-quality palliative care services.
Main Responsibilities- The successful candidate will be instrumental in coordinating safe and timely discharges of Palliative patients from hospital to their preferred place of care.
- Working collaboratively with the hospice team, the individual will assess patients in their own homes or inpatient environment to ensure they achieve their wishes at the end of life.
This role requires effective communication and collaboration with a multi-disciplinary team to offer support to patients, relatives, and healthcare professionals throughout the pathway.
Key duties include developing discharge plans in collaboration with hospitals, GPs, and community services, providing necessary care and advice, contributing to quality improvement initiatives, and ensuring smooth transitions of care provision.
An estimated salary of £35,000 - £45,000 per annum reflects the importance of this role in providing compassionate care to individuals facing incurable diseases.
Requirements- NMC Registration is essential for this position.
- A Disclosure and Barring Service check will also be required as part of the hiring process.
