Mendip Vale Medical Group is seeking a skilled Social Prescriber Link Worker to join our team in Bristol and South Gloucestershire. As a key member of our team, you will play a vital role in empowering patients to take control of their health and wellbeing by addressing the wider determinants of health.
Key Responsibilities
- Provide personalized information, advice, and support to patients, meeting their unique needs, circumstances, and preferences.
- Develop trusted relationships by giving patients the time to discuss what matters to them, fostering a sense of trust and confidence.
- Support and signpost patients through referrals of non-medical pathways, ensuring they receive the necessary care and resources.
- Co-produce personalized actions and goals to improve health and wellbeing, tracking progress and making adjustments as needed.
- Support the development of new community groups where there are gaps in services, promoting community engagement and social connections.

Person Specification
- NVQ Level 2 in Health or Social Care or other relevant professional academic qualification.
- Evidence of continued learning and personal development in the relevant field.
- Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities.
- An understanding of health, social care, and voluntary sector provision.
- Knowledge of the personalized care approach.
