Job Description:
We are seeking a motivated and compassionate Health and Wellbeing Coach to join our team at Bristol City Robins Foundation. The successful candidate will work in partnership with Bridge View Medical PCN to provide support to individuals with various health needs.

Key Responsibilities:
• Identify individuals who would benefit from health and wellbeing coaching interventions.
• Provide non-clinical support to patients, guiding them to change their lifestyle behaviours to promote better health and wellbeing.
• Take referrals from the wider practice team, including GPs, Practice Nurses, and Social Prescribers.
• Teach patients and carers to understand and manage their conditions through health coaching techniques.
• Support the development of personalised patient care and support plans, liaising with the GP practice team and specialist teams as needed.

Requirements:
• PCI-Accredited Tailored Health Coaching Course (desirable).
• Experience working in a public-facing setting, ideally in a culturally diverse community.
• Ability to motivate and educate patients to engage in dietary and lifestyle changes to benefit their health.
• Full UK driving license.
