Job Summary

We are seeking a highly motivated and compassionate Care and Support Worker to join our team in East Cornwall. As a Care and Support Worker, you will play a vital role in providing personalized care and support to our clients, enabling them to maintain their independence and quality of life.

Key Responsibilities
  • Provide personalized care and support to clients in their own homes, including personal care, meal preparation, and housekeeping.
  • Support clients to access community opportunities and engage in activities that promote their physical and mental well-being.
  • Work collaboratively with healthcare professionals, families, and other stakeholders to ensure seamless care and support.
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each client.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
Requirements
  • Full driving license and access to a vehicle.
  • Current right to work in the UK.
  • Understanding of reablement and care provision.
  • Excellent communication and interpersonal skills.
  • Basic IT skills.
  • Empathy and a passion for supporting others.
  • Excellent attention to detail and time management skills.
