CYP Access Team Practitioner

We are seeking a highly skilled and experienced professional to join our Access Team as a CYP Access Team Practitioner. The successful candidate will be responsible for providing clinical support to the team, screening referrals to CYP services, and liaising with peers to ensure the young person receives the right care at the right time.
Key Responsibilities- Provide clinical support to the team based on knowledge and experience as and when needed.
- Support the induction of new staff.
- Attend team meetings and away days where necessary.
- Complete audits as required.
- Support the implementation of service improvements & changes within the team setting.
- Act as Practice Supervisor or Assessor for student nurses in the team.
- Screen referrals to CYP services and liaise with peers to ensure the young person receives the right care at the right time.
- Provide cover on the Here4You line to triage calls and provide support to professionals and parents/carers.
- Complete thorough and holistic triage and risk assessments where necessary.
- Liaise with other professionals/services re. referrals and according to CYP needs.
- Recognition of own skills and professional capability to escalate cases as necessary.
- Gather feedback from children, young people, parents/carers and other professionals to enable service growth and development.
We are looking for a professional with the following qualifications and experience:
- RMN/RNLD/Social Worker/Occupational Therapist with current registration.
- Or educated to degree level in mental health/social care or another relevant subject or equivalent experience within the mental health field of working.
Desirable qualifications and experience include:
- Therapeutic interventions qualification.
- Experience of working with people with mental health needs, including those with emotional wellbeing needs.
- Experience in risk management of people with emotional wellbeing/mental health needs.
- Good knowledge of universal children's services.
- Good knowledge of children and young people's mental health and emotional wellbeing services.
