We are seeking a highly skilled Diabetes Care Specialist to join our team at Cambridgeshire & Peterborough NHS Foundation Trust.

Our clinical teams deliver various NHS services, including children, adult & older people's mental & physical health, forensic & specialist mental health, learning disabilities, primary care & liaison psychiatry, substance misuse, social care, research & development.
With a strong commitment to patient-centered care, we aim to provide advanced clinical practice in the field of diabetes care. Our ideal candidate will possess expertise in managing complex diabetes, providing specialist education to patients, nurses, and other healthcare professionals, and working collaboratively with multidisciplinary teams.
**Key Responsibilities**
- Assess, plan, implement, and evaluate programmes of care for patients with diabetes, providing specialist advice and actively seeking evidence-based practice.
- Work autonomously to manage a significant caseload of patients with complex diabetes, developing individualized programmes of care that meet their unique needs.
- Provide community nurse-led clinics within general practice, working proactively to prevent admission to secondary care.
- Participate in clinical supervision on a regular basis and be responsible for the management of and reporting on individual and team performance indicators.
**Requirements**
- Mandatory qualifications include First level registration ENB 928 or equivalent in diabetes nursing, ENB 998 or teaching certificate, and evidence of continuous professional development.
- Possession of a degree in a health-related subject, non-medical prescriber Diabetes educator certification, or counselling qualification is desirable.
**Salary:** £45,989 - £56,589 per annum (dependent on experience). This salary estimate may vary based on factors like location, industry standards, and specific job requirements.
