Job Description

As a Nurse Practitioner - Diabetes Care Specialist, you will play a vital role in providing expert care to patients with diabetes. This is an exciting opportunity to join our team at Cambridgeshire & Peterborough NHS Foundation Trust and make a real difference in the lives of our patients.

About the Role

  • You will work collaboratively with our multidisciplinary team to provide high-quality, individualized care to patients with diabetes.
  • You will be responsible for assessing, planning, implementing, and evaluating programmes of care for patients with complex diabetes.
  • You will work autonomously to manage a significant caseload of patients, providing specialist advice and actively seeking evidence-based practice to improve patient outcomes.
  • You will provide education to patients, nurses, and other healthcare professionals on diabetes management and self-care.

Requirements

  • You must have a first-level registration in diabetes nursing (ENB 928 or equivalent) and experience in managing complex diabetes cases.

  • You must have a good understanding of safeguarding issues and be able to interpret them accurately for all individuals.
