Adult Community Nurse

We are seeking a skilled and compassionate Adult Community Nurse to join our team at Bank Partners. As a key member of our community healthcare team, you will provide high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes.
Key Responsibilities:- Provide skilled nursing care to patients with complex needs, including assessments, planning, implementation, and evaluation of individual care programs.
- Work collaboratively with specialist nurses, allied health, and social care professionals to meet patient needs and prevent unnecessary A&E attendance or hospital admissions.
- Conduct first assessments and reviews, and develop and implement care plans that meet the unique needs of each patient.
- Work independently as part of the community day nursing team, covering a specific geographical area.
- Collaborate with the community day nursing team manager to ensure seamless care for patients.
- Registered General Nurse with a strong commitment to delivering high-quality patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team.
- Compassionate and empathetic towards patients and their families.
- Ability to work independently and as part of a team, with a flexible approach to meet the needs of patients and the organization.
- Full UK driving license with access to a car (business mileage allowance paid per mile).
