Job Overview

We are seeking a highly skilled and compassionate Registered General Nurse to join our Adult Community Day Nursing Service at Hounslow and Richmond Community Healthcare (HRCH). As a key member of our team, you will be responsible for delivering high-quality patient care to housebound individuals with complex needs, enabling them to remain in their own homes.

Main Responsibilities
  • Provide skilled nursing care to patients with complex needs, promoting independence and well-being.
  • Work collaboratively with the Community Day Nursing Team Manager to plan, implement, and evaluate patient care programs.
  • Conduct thorough assessments and reviews, developing and implementing individualized care plans.
  • Collaborate with specialist nurses, allied health professionals, and social care teams to ensure seamless care and prevent unnecessary hospital admissions.
  • Be flexible and adaptable to support a corporate approach to patient care, working across the Hounslow and Richmond areas.
Requirements
  • Registered General Nurse with a strong commitment to delivering high-quality patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Full UK driving license and access to a car, with business mileage allowance paid.
