About the Role

We are seeking a highly motivated and compassionate Assistant Practitioner to join our Child and Adolescent Mental Health Service in Great Yarmouth. As a valued member of our team, you will play a crucial role in supporting children, young people, and their families in achieving their full potential.

Key Responsibilities
  • Assess, plan, and deliver interventions in collaboration with the multidisciplinary team.
  • Engage with service users and their carers to promote person-centered care and support.
  • Work autonomously within defined boundaries, taking ownership of delegated tasks and responsibilities.
  • Contribute to the development and implementation of care plans, ensuring the needs of service users are accurately reflected.
  • Participate in multidisciplinary team meetings and contribute to the review of risk assessments.
Requirements
  • Foundation Degree in Health and Social Care or a related field.
  • Experience working with service users with mental health needs and within multidisciplinary teams.
  • Excellent communication and problem-solving skills, with the ability to work independently and as part of a team.
  • Understanding of mental health issues, risk assessment, and the Care Programme Approach.
