About the Role

We are seeking a highly skilled and dedicated Assistant Practitioner to join our Child and Adolescent Mental Health Service in Great Yarmouth. As a valued member of our team, you will play a crucial role in supporting children, young people, and their families in achieving their mental health goals.

Key Responsibilities
  • Assess, plan, and deliver interventions in collaboration with the multidisciplinary team.
  • Engage with service users and their carers to understand their needs and develop personalized care plans.
  • Work autonomously within clearly defined boundaries, taking ownership of delegated tasks and responsibilities.
  • Contribute to the development and implementation of care plans, ensuring a holistic approach to service user care.
  • Collaborate with external organizations to promote partnership working and ensure seamless care delivery.
  • Participate in multidisciplinary team meetings and contribute to the development of service user care plans.
  • Hold and manage an identified caseload, ensuring accurate and timely documentation of service user needs.
  • Contribute to the ongoing review of risk assessments, recognizing and responding to any changes in service user needs.
Requirements
  • Care certificate or relevant equivalent.
  • Foundation Degree in Health and Social Care/Mental Health or apprenticeship in H&SC/Mental Health L5.
  • Working experience with service users with mental health needs.
  • Working knowledge and understanding of the Care Programme Approach (CPA) process.
  • Ability to communicate widely in a clear manner.
  • Problem-solving skills.
  • Confident and approachable demeanor.
  • Good time management and ability to meet deadlines.
  • Understanding of mental health issues and risk assessment.
