About the Role

We are seeking a highly skilled and dedicated Assistant Practitioner to join our Child and Adolescent Mental Health Service in Great Yarmouth. As a valued member of our team, you will play a crucial role in supporting children, young people, and their families in achieving their mental health goals.

Key Responsibilities
  • Assess and plan interventions in collaboration with the multidisciplinary team to meet the needs of service users.
  • Deliver person-centered care, promoting the principles of recovery and social inclusion.
  • Work autonomously, taking ownership of your workload and making decisions within clearly defined boundaries.
  • Develop and maintain relationships with internal and external agencies, promoting partnership working and holistic care.
  • Participate in ongoing assessments of service users, identifying and responding to changes in their needs.
Requirements
  • Foundation Degree in Health and Social Care or Mental Health, or equivalent qualification.
  • Experience working with service users with mental health needs, within a multidisciplinary team.
  • Working knowledge and understanding of the Care Programme Approach (CPA) process.
  • Excellent communication and problem-solving skills, with the ability to work independently and as part of a team.
