Home Care Coordinator Role

Home Instead is seeking a skilled and compassionate Care Coordinator to join our team in Ossett. This part-time role will involve coordinating care services for clients, collaborating with care professionals, and maintaining accurate records in our electronic scheduling system.
About the Role- Care Coordination: Develop and maintain efficient care schedules considering client needs, care professional availability, and logistical factors.
- Liaising with Clients and Care Professionals: Ensure optimal matching of care services and prompt response to schedule changes.
- Staffing and Resource Management: Collaborate with recruitment and care teams to maintain appropriate staffing levels and resource care packages.
- Record Keeping: Maintain accurate client and care professional information in our electronic scheduling system.
This role requires strong organisational, time management, and communication skills, as well as proficiency in IT systems, including Microsoft Office and CRM software. The successful candidate will have a proven ability to work calmly and accurately under pressure, with strong problem-solving skills.
