Community Nurse for Children with Disabilities

About the Role
The Community and School Nurse will be a proactive nurse who works flexibly across the community and in our special schools delivering holistic, family-centered care for children and young people with complex health needs and disabilities.
This is a dynamic role encompassing assessment, planning, delivery, and evaluation of clinical nursing care. The post holder will provide comprehensive support for children/young people and their families, including signposting to specialist services.
Key responsibilities include:
- Delivery of specialist nursing care, including administration of medication, suctioning, tracheostomy care, gastrostomy management, and enteral feeding.
- Assessment, planning, delivery, and evaluation of clinical nursing care.
- Providing health advice for education, health, and care plans.
- Nursing sleep service - supporting children with sleep difficulties, reviewing sleep medication, and offering support and strategies to parents.
- Mentoring, supporting, and supervising junior staff and student nurses.
The post holder will be given the opportunity to develop a range of skills and knowledge within a diverse setting.
