Specialist Nurse for Children with Disabilities

About the Role
The Community and School Nurse will be a proactive nurse who works flexibly across the community and in our special schools delivering holistic, family centered care for children and young people with complex health needs and disabilities.
This is a dynamic role encompassing assessment, planning, delivery and evaluation of clinical nursing care. This will also involve delivery of specialist nursing care, including but not limited to: administration of medication, suctioning, tracheostomy care, gastrostomy management and enteral feeding, seizure management.
The post holder will provide comprehensive support for children/ young people and their families, including signposting to specialist services. They will deliver specialist training for families and care support staff, attend multi-professional meetings, including child protection case conferences.
They will also provide health advice for education, health and care plans, nursing sleep service- supporting children with sleep difficulties, reviewing sleep medication and offering support and strategies to parents.
The post holder will be mentoring, supporting and supervising junior staff and student nurses.
About the Team
The Children's Community Nursing Team, based at The Moor Lane Centre, Chessington, delivers specialist nursing care to children and young people with SEND in Dysart and Bedelsford special schools, at home and within community services.
The nursing team forms part of the Integrated Service for Children with Disabilities (ISCD), working across health, education and social care.
