Nursing Associate Specialist

We are seeking a highly skilled and compassionate Nursing Associate to join our Dementia Outreach Service at Southern Health NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality care to individuals living with dementia and their families.
About the RoleThe successful candidate will be responsible for conducting comprehensive assessments of needs and risks, planning and delivering care to individuals requiring an acute and intensive response, and collaborating with service users, carers, and other services to ensure seamless care coordination.
Key Responsibilities- Conduct thorough assessments of individuals' needs and risks, identifying areas for improvement and developing effective care plans.
- Deliver high-quality care to individuals living with dementia, ensuring their physical, emotional, and social needs are met.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure comprehensive care coordination.
- Develop and maintain strong relationships with service users, carers, and families, providing emotional support and guidance as needed.
- Participate in regular team meetings and contribute to the development of the service, ensuring continuous improvement and best practice.

Requirements- RMN or Allied Health Professional (Mental Health) qualification.
- Experience in assessment, risk management, and crisis intervention.
- Strong communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
- Ability to work flexibly, including evenings and weekends, to meet the needs of service users.
- Valid driving license and access to a vehicle.
