We are seeking a highly skilled and experienced Mental Health Practitioner to join our CAMHS Initial Assessment Team in Oldham. As a key member of our team, you will play a vital role in assessing and formulating treatment plans for young people, working closely with a multi-disciplinary team to provide high-quality care.
Key Responsibilities- Conduct comprehensive mental health assessments of young people, identifying their needs and developing effective treatment plans.
- Work collaboratively with a range of professionals, including nurses, psychologists, social workers, and psychiatrists, to provide a comprehensive service.
- Develop and maintain strong relationships with young people, their families, and carers, ensuring that their needs are met and their voices are heard.
- Contribute to the development and implementation of service plans, ensuring that they are aligned with the needs of the local community.
- Participate in regular team meetings and contribute to the development of the team's strategic plan.
- Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent qualification, with active registration with the relevant regulatory body.
- Significant post-qualification experience in community mental health services, to postgraduate diploma level.
- Proven experience of case management of young people, including assessment, formulation, and treatment planning.
- Excellent communication and interpersonal skills, with the ability to work effectively with a range of professionals and stakeholders.
- Ability to work autonomously, with a high degree of independence, and as part of a team, with a strong focus on collaboration and partnership working.
