Clinical Pharmacist

Central London Community Health Trust is seeking a highly motivated and experienced Clinical Pharmacist to join our Medicines Management team. The successful candidate will be responsible for providing expert advice and support on medicines management to healthcare professionals across the Trust.
Main Responsibilities
Provide professional expertise, advice, and support on medicines management to healthcare professionals across the Trust.
Contribute to the clinical governance of the Trust through interventions that optimise the management of medicines in all relevant areas of practice.
Support and deliver a high-quality clinical pharmacy service and medicines optimisation service to patients in Community Hospitals and ensure an integrated approach to the management of medicines and continuity of pharmaceutical care across the interface.
Lead, manage, and develop staff within the team.

Person Specification
Vocational Masters Degree in Pharmacy.
Professional registration with the General Pharmaceutical Council (GPhC).
Postgraduate Clinical Pharmacy Certificate qualification or equivalent.
Postgraduate Clinical Pharmacy Diploma qualification or equivalent.
Takes personal responsibility for Continuing Professional Development as evidenced by portfolio.
Uses reflective practice for personal and professional development.
Desirable
Independent Prescriber or working towards qualification.

Flexibility in approach to meet service needs and ability to support complex changes.
