About the Role

We are seeking a highly motivated and experienced Respiratory Physiotherapist to join our Community Respiratory Service in Hounslow. As a key member of our team, you will play a vital role in providing high-quality care to patients with respiratory conditions.

Main Responsibilities
  1. Contribute to the care provision and review of respiratory patients, promoting recovery and minimising risks through a multi-disciplinary approach.
  2. Provide direct care for patients with respiratory conditions in the community, including pulmonary rehabilitation and early hospital supported discharge.
  3. Develop and implement individualised care plans, working closely with patients, families, and healthcare professionals.
  4. Collaborate with the multidisciplinary team to deliver primary care education and support, ensuring seamless care transitions.
  5. Contribute to the development of the Community Respiratory Service, identifying opportunities for improvement and implementing changes to enhance patient outcomes.
Requirements
  • Registered Respiratory Physiotherapist with a strong background in respiratory care.
  • Experience in community-based respiratory care, with a focus on pulmonary rehabilitation and airway clearance.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
  • Valid driving license and access to a vehicle.
