Job Title: Band 5 Community Respiratory Physiotherapist

We are seeking a highly motivated and experienced Physiotherapist to join our Community Respiratory Service in Hounslow. As a Community Respiratory Physiotherapist, you will play a vital role in providing high-quality care to patients with respiratory conditions in the community.

Main Responsibilities:
  • Contribute to the care provision and review of respiratory patients, promoting recovery and minimising risks through a multi-disciplinary approach.
  • Provide direct care for patients with respiratory conditions in the community-based Oxygen and Respiratory Case Management Team, including Pulmonary Rehabilitation and early hospital supported discharge.
  • Develop and implement individualised care plans, working closely with patients, families, and healthcare professionals to achieve optimal outcomes.
  • Collaborate with the multi-disciplinary team to deliver high-quality care, ensuring seamless transitions between hospital and community settings.
Requirements:
  • Registered Physiotherapist with a strong background in respiratory care.
  • Experience working in community or hospital settings, with a focus on respiratory care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality care.
