Job Summary: We are seeking an experienced Physiotherapist to join our Community Rehabilitation Team. The successful candidate will work with patients to develop and implement personalized rehabilitation plans, promoting independence and improving quality of life.

Key Responsibilities:

  • Conduct assessments and develop rehabilitation plans for patients with complex physical and mental health needs
  • Deliver evidence-based physiotherapy interventions to promote independence and improve quality of life
  • Work collaboratively with multidisciplinary teams to achieve patient-centered outcomes
  • Participate in quality improvement initiatives and contribute to the development of best practice in physiotherapy

Requirements: The successful candidate will have:

  • Registration with the Health and Care Professions Council (HCPC)
  • Postgraduate experience in physiotherapy, preferably in a community rehabilitation setting
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work independently and as part of a team, with a flexible approach to work and a commitment to delivering high-quality patient care
