Job Title: Band 5 Community Respiratory Physiotherapist

We are seeking a highly motivated and experienced Community Respiratory Physiotherapist to join our team at West London NHS Trust. As a key member of our Community Respiratory Service, you will play a vital role in providing high-quality care to patients with respiratory conditions in the community.

Main Responsibilities:
  • Contribute to the care provision and review of respiratory patients, minimizing risks and promoting recovery through a multi-disciplinary approach.
  • Provide direct care for patients with respiratory conditions in the community-based Oxygen and Respiratory Case Management Team, including pulmonary rehabilitation and early hospital-supported discharge.
  • Develop and implement individualized care plans, promoting self-management and preventing unplanned hospital admissions.

Person Specification:
  • RGN or Physio qualifications
  • Completion of Assessors training
  • Post-registration nursing course in respiratory care
  • Knowledge and understanding of healthcare needs in a multicultural society
  • Knowledge of health promotion activities and strategies
  • Knowledge of respiratory medicines management
  • Experience of working with/ in primary care
Requirements:
  • Motivated to further personal and professional development
  • Must drive and have a clean driving license
