About the Role:
- Deliver high-quality nursing services to patients in their homes, ensuring compliance with all relevant regulations and professional standards.
- Assist in providing Patient Support Programmes, including education and training for patients and caregivers.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of patient needs and provide personalized care solutions.
- Ensure accurate recording of patient care activities and maintain up-to-date documentation.
Requirements:
- Degree in Nursing or related field.
- Valid registration with the NMC.
- Proven experience in homecare or similar settings.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Salary: £40,000 - £55,000 per annum (dependent on experience).
