Job Overview

We are seeking a highly skilled and dedicated individual to join our team as a Care Coordinator. As a key member of our Community Mental Health Team, you will play a vital role in providing integrated health and social care to individuals with moderate to severe mental health problems.

Key Responsibilities:

  • Coordinating, commissioning, and delivering individual packages of care for a caseload of service users
  • Assessing needs, formulating care plans, and implementing evidence-based interventions
  • Liaising with multi-disciplinary teams, agencies, and service users to ensure effective care delivery
  • Supervising Community Support Workers and peers as appropriate
  • Maintaining accurate, up-to-date electronic records

Requirements:

  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with care coordination experience
  • Experience working with individuals with severe and enduring mental health issues
  • Effective risk assessment skills and ability to complete funding applications
