We are seeking a highly skilled and dedicated Community Sister to join our Intermediate Care Team at First Community Health and Care. As a key member of our team, you will play a vital role in providing high-quality care and support to patients in their own homes.
Key Responsibilities- Assess the health and social care needs of patients and their carers within the community, aiming to prevent hospital admissions and facilitate discharge home.
- Work closely with Physiotherapists, Occupational Therapists, and Rehabilitation Assistants to provide comprehensive care and support to patients.
- Act as a role model for junior team members and contribute to the promotion of an environment that encourages and supports development of staff using evidence-based nursing practice.
- Utilize a variety of strategies to support patients at home and communicate effectively with family, carers, the multidisciplinary team, voluntary organizations, and other agencies to facilitate rehabilitation and promote independence.
- Relevant NMC qualification and experience of at least 2 years post-graduate experience.
- Excellent communication and interpersonal skills, with the ability to organize workload, delegate, and prioritize tasks.
- Good knowledge of health and safety and risk management, with basic IT skills and a flexible approach to service delivery.
