Job Title: Service Coordinator

Job Summary:

We are seeking a dedicated and compassionate Service Coordinator to join our team at Adullam Homes Housing Association Ltd. in Chesterfield. As a key member of our team, you will play a vital role in supporting young people who have left foster care, care homes, or are seeking asylum.

Key Responsibilities:

  • Develop and deliver person-centred support plans to suit the needs of young people accessing our service.
  • Guide young people to gain skills such as budgeting, maintaining their accommodation, and cooking.
  • Help young people make safe choices, look after their mental and physical health, and engage them in education.
  • Contribute to the maintenance and development of the Association's values, culture, and ethos.
