Job Description:

We are seeking a highly skilled and experienced Social Worker Level 3 to join our Adult Social Care team. The successful candidate will be responsible for assessing, planning, and implementing support for adults with varying needs, ensuring they receive the highest quality of care and support.

Key Responsibilities:
  1. Conduct comprehensive assessments of individuals' needs, including physical, emotional, and social requirements.
  2. Develop and implement personalized support plans to meet the unique needs of each individual.
  3. Work collaboratively with other healthcare professionals, family members, and carers to ensure seamless care and support.
