As a key member of our multidisciplinary team, you will work closely with patients, carers, and other healthcare professionals to provide high-quality care and support.
Key Responsibilities- Manage a caseload of patients with organic and functional presentations under CPA and NCPA
- Assess and manage risk
- Use Dialog+ and Recovery Care Planning with service users and carers
- Communicate effectively through notes on Lorenzo and letters to Service Users, GPs, and other involved parties
- Administer depot medication
- Drive independently for work-related purposes
