Job Title: Senior Community Mental Health Nurse

We are seeking an experienced and skilled Senior Community Mental Health Nurse to join our Adult Community Mental Health Team in Newmarket and surrounding areas.

Main Responsibilities:
  • Manage a caseload of patients with mental health care needs and provide direct care with minimal supervision.
  • Work within a multi-disciplinary team environment to ensure continuity of practice and effective care planning.
  • Assess and plan treatment for patients, working collaboratively with them to optimize treatment plans.
  • Provide risk assessments and screenings for patients, identifying suitability for potential discharge in conjunction with the Senior Nurse.
  • Advise the Responsible Medical Officer and other healthcare professionals of any changes in patient conditions or circumstances.
  • Form working partnerships with other care providers to ensure seamless care delivery.
  • Enable and empower service users and carers to make informed decisions about their care.
Requirements:
  • Experience working within a multi-disciplinary team environment.
  • Previous community experience, preferably in adult mental health.
  • Nursing degree or equivalent, with evidence of social care or health-related study.
  • Ability to travel independently and work flexibly.
