Job Summary

We are seeking a highly skilled and compassionate Senior Community Mental Health Nurse/Practitioner to join our dedicated team at Norfolk & Suffolk Foundation NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with mental health needs in our community.

About the Role

As a Senior Community Mental Health Nurse/Practitioner, you will be working with a diverse range of clients, including older adults and adults with complex mental health needs. You will be responsible for assessing, planning, implementing, and evaluating care and treatment plans to meet the individual needs of your clients. You will also be working closely with other healthcare professionals, including GPs, social workers, and other community mental health teams, to ensure seamless care and support.

Key Responsibilities
  • Assess and plan care and treatment for clients with mental health needs
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Provide high-quality care and support to clients in their own homes or in community settings
  • Work collaboratively with other healthcare professionals to ensure seamless care and support
  • Conduct regular reviews and assessments to ensure the effectiveness of care and treatment plans
Requirements

To be successful in this role, you will need to have:

  • Registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse
  • Experience of working in community mental health settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong assessment and planning skills
