We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our multidisciplinary team as a Care Coordinator. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality community mental health services to individuals and their families.
Key Responsibilities- Assess, implement, and evaluate care and treatment plans for individuals on the caseload, working in conjunction with service users, carers, and other healthcare professionals.
- Work autonomously, but under the indirect supervision of a Band 7 registered nurse/team manager, to deliver a supportive service for those under the care of the CMHT.
- Participate in multidisciplinary team meetings and contribute to the development of care plans.
- Provide mental health assessments and support to service users in the absence of their care coordinator.
- Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to provide comprehensive care.
- Registered Mental Health Nurse with a postgraduate qualification (diploma, degree, or higher qualification) or evidence of working towards the same.
- Experience of working at Band 5 within a multidisciplinary team framework in Acute Adult Mental Health Services.
- Experience of joint working with other agencies/services.
- Ability to work autonomously, communicate effectively with service users, and engage with a range of people from statutory and non-statutory agencies.
- Ability to assess risk and respond appropriately using evidence-based literature.
- Knowledge of evidence-based practices within a mental health setting, Health Board policies and procedures, medication, risk assessment, and clinical audit/governance.
